The EHD will accommodate your schedule and make a one-time exception for scheduling the inspection without the required 48 -hours advance notice. In the future the 48 hours notification to the EHD will be required prior to any inspection and testing. Class II violations: 104, 105, 108, 109, 201, 202, 205, and 206- 104. Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents are required to be submitted annually. Complete and submit a copy of the financial responsibility by July 27, 2013. 105. UST facility and tank form on file with the EHD (Operating Permit Application -Facility Information) not current. Any change of information on these forms must be submitted to the EHD within 30 days of the changes. Immediately complete and submit a copy of the facility form to the EHD. 105. UST facility form is lacking BOE Maintenance Fee Account Numbers or BOE number is incorrect. Immediately complete and submit a copy of the facility form to the EHD. 108. Secondary containment testing was last performed on January 13, 2009 and was due by January 2012. Testing has not been completed and is 5 months past due. Secondary containment testing is required once every 36 months. Immediately schedule this test and provide 48 hours notification to the EHD. L� 109. The functionality of all STP, UDC and annual sensors, and the positive shut down and fail safe were verified on October 25, 201(SR0065801) and a test report has not been submitted to the EHD. A copy of the test report must be stibmitted within 30 days of the tests. Immediately submit a copy of the test report to the EHD. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
