Alarm logs, maintenance invoices, prior Monitoring System Certification, and secondary testing <br />containment results were not found on site. These records shall be kept on site for at least 3 years. <br />Immediately locate and maintain the above records on site and submit copies to the EHD by July 27, <br />2013. If you are unable to locate the prior years test results you can make an appointment with the EHD <br />for a file review. <br />202. Approved copies of the monitoring and response plans were not found on site. Copies of these <br />plans shall be retained on site at all times. Immediately locate and retain a copy on site, if unable to <br />locate these documents you can make an appointment with the EHD for a file review. <br />206. The designated operator's employee training log was not available for inspection. The designated <br />operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by July 27, 2013. <br />205. The designated operator monthly inspection reports were not found on site. Designated operator <br />monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure <br />that copies of the previous twelve months of designated operator monthly inspection reports are <br />maintained on site. Submit copies to the EHD by July 27, 2013. <br />NOTES: Complete and submit a copy of the Return to Compliance Certification form with a statement of <br />how each violation was corrected, including supporting documentation, by July 27, 2013. Log in to the <br />California Environmental Reporting System (CERS ID 10180675) and upload all the required <br />documentation. <br />PROVIDED: Return to Compliance Certification, Owner Statements of Designated UST Operator, 2013 <br />Training Classes, CERS Lead User Authorization Form, Operating Permit Application- Facility <br />Information, and Certification of Financial Responsibility Form. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received By: <br />Title: <br />67k�14a AcuaL <br />z� q , <br />EHD 23-02-003 Rev 04/19/12 1 V CONTINUATION FORM <br />
