Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility to the EHD within 30 days of receiving this report. <br />105. Tank forms on file with the EHD are not current. Any change of information on these forms must be <br />submitted to the EHD within 30 days of the changes. Immediately complete and submit a copy of the <br />facility and tank forms to the EHD. Complete a separate tank form for each tank. <br />106 & 202. Current approved copies of the monitoring and response plans were not found on site. The <br />monitoring and response plans must be current and approved by the EHD. Complete and submit a copy <br />of the monitoring and response plans for approval by the EHD within 30 days of receiving this report. An <br />approved copy will be returned for your records. <br />303. The annular sensor on both the tanks and the Bravo float/ chain at disp 1 for the diesel pipe is not <br />working properly to ensure continuous monitoring of the tank system as required by CCR 26301 The <br />420 sensors were cracked and the Bravo float had water in it. Correct immediately by having a properly <br />licensed, trained, and certified contractor replace the failed component with a functional component (LG <br />113-x listed, if applicable) and obtain a permit within one business day from the EHD. If the failed <br />component can't be replaced immediately, there is a possibility that the UST system may be red tagged <br />to prevent fuel inputs. The sensors were replaced tested and passed (87 tank annular sensor new SR# <br />915687, old 0678847. The 91 tank annular sensor new SR# 851749, old 073494). The Bravo float at <br />dispenser 1 was replaced, tested and passed. Permit must be obtained next business day. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Inspector.
