The Bravo float in the UDC at dispenser 1 failed to stop the flow of product at the dispenser when <br />tested due to water in the float. All monitoring equipment shall be maintained to activate an audible and <br />visual alarm or stop the flow of product at the dispenser when it detects a leak. Correct immediately by <br />having a properly licensed, trained, and certified contractor replace the failed component with a <br />functional component (LG 113-x listed, if applicable) and obtain a permit within one business day from <br />the EHD. If the failed component can't be replaced immediately, there is a possibility that the Diesel <br />UST system may be red tagged to prevent fuel inputs. <br />313. The 87 grade UST direct -bury spill container failed when tested. All spill containers shall have a <br />minimum capacity of five gallons and be capable of containing a spill or overfill until it is detected or <br />cleaned up. Immediately discontinue deposition of petroleum into this tank until the component is <br />repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection <br />of the EHD. If the spill container can't be replaced immediately, there is a possibility that the 87 grade <br />UST system may be red tagged to prevent fuel inputs. <br />Notes <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation to the EHD within 30 days of receiving <br />this report. <br />Secondary containment testing was performed on 5/14/12 and passed. <br />There is a new operator at the facility and the Tanks forms, Monitoring plan, Response plan, Financial <br />Responsibility Certification, Designated operator Owner Statement and New EPA ID number for the <br />hazardous waste disposal was not available yet. <br />Facility will not be open for business for another 3 weeks. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received By: �1 � <br />/ �'� L'� G^ I o C <br />Title: <br />EHD 23-02-003 Rev 07/12/11 CONTINUATION FORM <br />
