An EPA ID number application was <br /> provided. <br /> On January 13, 2005, Ms. Dina Craw, REHS, EHD, called and spoke with Mr. Roshan <br /> Minhas, owner, and explained to him that the 89-octane spill container needed to be <br /> repaired immediately. <br /> On February 17, 2005, Mr. Shih sent a follow up letter asking for a valid EPA ID number <br /> (Attachment 5). An EPA ID number application was included. <br /> On March 14, 2005, Mr. Minhas submitted an EPA ID number, CAL00286016, but it was <br /> the EPA ID number for another store he owned in Manteca, Tiger Express Stores. <br /> On April 7, 2005, Mr. Shih called Mr. Minhas and told him that the EPA ID number he <br /> submitted was for his Manteca store and that the Grant Line store needs its own <br /> number. Mr. Minhas requested that Mr. Shih fax him a copy of the EPA ID number <br /> application. <br /> On April 28, 2005, Mr. Shih checked the Department of Toxic Substances Control <br /> (DTSC) Hazardous Waste Tracking System (HWTS) website and found a valid EPA ID <br /> number for this facility. Mr. Shih printed and filed the HWTS EPA ID profile (Attachment <br /> 6). <br /> On July 29, 2005, a permit application was submitted to this office by Mr. Minhas to <br /> replace all three spill containers as part of the Enhanced Vapor Recovery (EVR) <br /> upgrade. Wright Environmental was indicated as the contractor that will perform the <br /> work. This permit was approved by Mr. Willy Ng, REHS, EHD, on August 3, 2005 <br /> (Attachment 7). <br /> On August 29, 2005, Mr. Ng received a call from Shelly with Wright Environmental <br /> Services, who stated that Mr. Minhas decided to have another contractor perform the <br /> EVR upgrade. <br /> On November 7, 2005, Mr. Ng received a call from Mr. Robert Barnhart with Wright <br /> Environmental Services, who stated that the owner has rehired them to perform the EVR <br /> upgrade. An inspection time for corrosion protection was scheduled. <br /> On November 8, 2005, Mr. Ng verified the presence of corrosion protection on the spill <br /> containers and risers. <br /> On November 9, 2005, Mr. Ng verified the length of the drop tubes and that the <br /> mechanical overfill prevention was set at 95% of capacity. <br /> On November 10, 2005, Mr. Dennis Catanyag, REHS, EHD, witnessed the testing of the <br /> new spill containers. All passed. <br /> On December 30, 2005, Mr. Ng performed a routine UST inspection (Attachment 8) and <br /> witnessed the annual monitoring system certification, line leak detector testing, and spill <br /> container testing (Attachment 9). The monitoring system certification and line leak <br /> detector testing were performed eight months late, the October and November 2005 <br /> designated operator reports and records of employee training by the designated <br /> 2 <br />
