Mr. Ng did not note that secondary containment testing <br /> was last completed on April 15, 2002, was due by April 2005, and had not been <br /> performed. <br /> On January 30, 2006, Mr. Ng sent a follow up letter to Mr. Minhas (Attachment 10) <br /> asking for all the items requested on the December 30, 2005, inspection report. Nothing <br /> was submitted as of this date. <br /> On March 7, 2006, Mr. Ng performed a follow up inspection because none of the items <br /> requested on the December 30, 2005, inspection report were submitted. Mr. Ng found <br /> during the inspection that none of the designated operator information missing from the <br /> last inspection were on site and issued another report requesting items be submitted <br /> immediately. Again, Mr. Ng did not note that secondary containment testing was past <br /> due (Attachment 11). <br /> On March 8, 2006, Mr. Chuck Hill of Champion Precision Testing submitted a copy of <br /> the October and November 2005 designated operator reports and last training log on <br /> behalf of Mr. Minhas (Attachment 12). On March 14, 2006, Mr. Minhas submitted the <br /> return to compliance certification form with no supporting documentation (Attachment <br /> 13). <br /> On March 20, 2006, secondary containment testing was completed eleven months late <br /> and all components tested passed. A test report was submitted to this office <br /> (Attachment 14). <br /> On March 13, 2007, Mr. Ng performed a routine UST inspection and found that the <br /> annual monitoring system certification, line leak detector testing and spill container <br /> testing had not been performed and were three months past due. Current financial <br /> responsibility documents and a current designated operator statement have not been <br /> submitted to this office. The May 2006, August 2006, September 2006, October 2006, <br /> and January 2007 designated operator inspection reports were not found on site. An <br /> inspection report was left with Mr. Harminder Parhar, who stated he was Mr. Minhas's <br /> business partner (Attachment 15). <br /> On March 28, 2007, Mr. Ng witnessed the annual monitoring system certification, line <br /> leak detector testing, and spill container testing. All equipment that was tested passed. <br /> A test report was submitted by Champion Precision Testing on April 9, 2007 (Attachment <br /> 16). <br /> On May 11, 2007, Mr. Ng sent a follow up letter to the Grant Line Valero (Attachment 17) <br /> asking for all the items requested on the March 13, 2007, inspection report. None of the <br /> items were previously submitted. On May 24, 2007, Mr. Ng returned a call from Mr. <br /> Minhas who stated he would get with his partner and work on the items required on the <br /> last inspection. <br /> On July 13, 2007, a designated operator statement was submitted to this office by Tank- <br /> Tek Environmental Corporation on behalf of Mr. Parhar (Attachment 18). <br /> On March 12, 2008, a designated operator statement dated March 6, 2008, was <br /> submitted to this office by Afforda-Test on behalf of Mr. Minhas (Attachment 19). <br /> 3 <br />
