An emergency coordinator and modified contingency plan information is lacking. There must be i <br /> at least one emergency coordinator on site or on call to coordinate emergency response measures, and <br /> the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator-, location of fire extinguishers, spill control equipment, and if present, fire alarm-, and the <br /> phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br /> emergency coordinator and post the required information by a phone. A form is provided that can be <br /> used for this purpose. Submit proof of correction to the EHD by June 10, 2013. <br /> 812. Copies of universal waste disposal records were not found on site. A generator of universal waste <br /> (batteries, mercury containing equipment, lamps/bulbs, electronic devices, CRTs, CRT glass, aerosol <br /> cans) shall keep a record of each shipment of universal waste on site for at least three years. The <br /> record may take the form of a log, invoice, manifest, bill of lading, or other shipping document, and shall <br /> include the following: <br /> - Name & address of destination facility <br /> - Quantity & type of universal waste <br /> - Date of departure/shipment <br /> Begin this practice immediately. <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by June 10, 2013 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> Note: (corrected on site) <br /> The eyewash station was not functional at the time of inspection. All facility communications and alarm <br /> systems, fire protection equipment, spill control equipment, and decontamination equipment, shall be <br /> tested and maintained as necessary to assure its proper operation in time of emergency. Mr. Harrell <br /> turned the water on to the eyewash and tested it for proper operation at the time of inspection. <br /> This facility collects and accepts x-ray fixer waste from other facilities, sometimes on manifest, removes <br /> the silver from the waste, and packages the recycled fixer for reuse.
