PHS/EHD did not receive the letter until October 26, 1992. In addition PHS/EHD i <br /> was not notified verbally of the activity so PHS/EHD staff was not present. In the future please provide <br /> PHS/EHD staff notice 48 hours prior to any site activity so they may be present.
