SANJ O A Q U I N Environmental Health Department <br /> Complaint Record Daily Time and Activity Summary from 1121/2013 to <br /> 112'V2018 for COPAUHORN located at <br /> 16888 SOUTH MCKINLEY AVENUE <br /> Activity Date Recorded by <br /> 11/20/2018 With Mai Vue, EHST. PE 2546 BAKER <br /> 144 min SC 544 <br /> Leyna Huynh, Lead Senior REHS, received a page at 7:35am and handed it off to <br /> me. I called SJGH at 468-6000. They reported a diesel exhaust fluid (DEF) spill <br /> at Super Store Industries located at 16888 McKinley Avenue in Lathrop. The <br /> release was reported by Kelly Durrer, the Safety and Risk Manager, for Super <br /> Store Industries (209-858-3370). <br /> 1 called Kelly Durrer about the release. She said that they were switching out totes <br /> of DEF and the forklift punctured the tote and released DEF into the secondary <br /> containment. That secondary containment was filling up so they tried to replace it <br /> with another spill pallet and the spill pallet was punctured by the forklift as well. <br /> Approximately 250 gallons of DEF was release with 45 gallons to the ground and <br /> the rest within secondary containment. The spill has been contained and no <br /> waterways or storm drains were impacted. ACT Advanced Chemical will be <br /> arriving to pump out the secondary containment and taking away the clean-up <br /> material. Responsible party for this release was identified as Smart <br /> Transportation. Kelly Durrer stated that someone will be on site so that I can get <br /> their information. <br /> Generated a complaint. <br /> Spill report#18-7861. <br /> Left at 1:00pm to verify the clean-up. Arrived on site at 1:25pm. Met with Kelly <br /> Durrer and Al Hollander with Smart Transportation. Kelly Durrer said that Super <br /> Stores Industries will be the responsible party and they will send all billing to Smart <br /> Transportation. Site has already been cleaned up. Spill occurred on the south <br /> side of the maintenance shop on the outside. No waterways or storm drains were <br /> impacted by the release. Clean-up was conducted by Super Store Industries. <br /> One white 1x1 yard bag of solid waste was generated and four 55 gallon blue <br /> plastic drums of waste. Waste will be disposed of next week as hazardous waste. <br /> Left site at 1:45pm and returned to office. <br /> Finished notice to abate and emailed it to Kelly Durrer at kdurrer@ssica.com. <br /> Completed emergency response record, prop 65 and site map. <br /> 11/21/2018 Finished photo log. This complaint is connected with PR0514244. Please check PE 2546 BAKER <br /> 18 min for disposal of four 55 gallon drums and 1 x1 yard bag of waste generated during SC 544 <br /> this clean-up during the next routine hazardous waste inspection. <br /> Abated complaint and put on Garrett Backus', Lead Senior REHS, shelf for review. <br /> Page 1 of 1 <br />
