SAN )O A Q U I N Environmental Health Department <br /> Complaint Record Daily Time and Activity Summary from 7/20/2018 to <br /> 1730/2018 for CORNGBBKR located at <br /> 1630 E HAMMER LN. STOCKTON, CA <br /> Activity Date Recorded by <br /> 09/25/2018 At 08:20am today, Elianna Florido with EHD discussed with me following up on PE 2546 SAEED <br /> 198 min this complaint. I reviewed the complaint. I left the office at 11:00am to inspect the SC 544 <br /> spill. I arrived onsite at 11:25am. I met with Charlie Morris,Asst. Manager at <br /> 11:30am. I inspect the impacted areas ( Pavement and storm drain), and took <br /> photos. Costco attendant cleaned-up the pavement, and stored the waste into a <br /> 55 gallon waste absorbent drum. Patriot Environmental cleaned-up the storm drain <br /> basin, and generated 55 gallon drum of liquid waste. Both drums are stored <br /> onsite. Per Charlie Morris, Costco Asst. Manager, the spill did not leave the storm <br /> drain basin, and storm drain flows to an oil water separator. Absorbent Boom was <br /> placed inside the pipe that flows from the storm drain to the oil water separator, <br /> and it showed no sign of residue or contamination. City of Stockton were onsite, <br /> and verified that the spill did not impacted the adjacent storm drain, and were <br /> satisfied with the clean-up. <br /> I emailed the notice to abate to Charlie and explained it to her. When the City of <br /> Stockton arrived onsite; I joined them for the inspection, and updated the notice to <br /> abate; I emailed a copy of the updated notice to abate to Charlie. Charlie <br /> discussed with me the spill reporting requirements, and told her to follow their <br /> emergency response plan; also told her all the spill/release are reportable, and <br /> emailed her a copy of SJC EHD notification guidance. I left the site at 1:00pm. <br /> When I returned to the office I started filling out the information for prop 65, and <br /> emergency response records. I completed the narratives. I updated Garrett <br /> Backus on the status of the incident. <br /> 09/27/2018 1 completed the emergency response records and printed a copy. I uploaded the PE 2546 SAEED <br /> 72 min incident photos to my computer, and completed the photo log. I received a copy of SC 544 <br /> Notice on Warning from Akin Fayehun with City of Stockton. I reviewed it, and <br /> printed it to attach to the report. <br /> 10/08/2018 1 received an email from Charlie Morris with response action letter, and manifest s. PE 2546 SAEED <br /> 24 min I reviewed and printed a copy to attach with the report. I replied to the email to SC 544 <br /> send the final manifest when they receive them. <br /> 10/22/2018 Received the final manifest for the contaminated waste solid. I reviewed the PE 2546 SAEED <br /> 12 min manifest, printed a copy, and attached to the report. I emailed Charlie Morris to SC 544 <br /> ask for the manifest for the other drum (Liquid). <br /> Page 2 of 3 <br />
