UW lamps were not stored in a closed container & in a manner that prevents <br /> breakage and release to the environment. Correct immediately by storing UW lamps per <br /> 66273.33 (b) CCR. UW lamps are now stored in containers in a locked and secured <br /> compartment. Please see attached photograph. <br /> 82. UW lamps were not labeled to identify type of universal waste. Correct immediately <br /> by labeling each shipment of UW per 662733.34 CCR. UW lamp containers are now <br /> being labeled to identify the type of UW it contains. Please see attached photograph. <br /> 83. Stored universal waste onsite for more than I year or was unable to demonstrate the <br /> length of time UW has accumulated from the date it became a waste or was received, <br /> Correct immediately by disposing of the UW lamps and send copies of disposal records <br /> to SJC EHD by 07-18-09. All UW lamps were properly disposed of on 06-25-09 by <br /> American Valley Waste Oil. Please see attached records. <br /> 85. Disposal records for UW lamps were not observed. Correct immediately by <br /> maintaining records of each shipment of UW per 66273.39 CCR. UW lamps were <br /> properly disposed of on 06-25-09 by American Valley Waste Oil. Please see <br /> attached records. <br /> 89. Two 55-gallon drums that used to contain Haz Waste/Haz Mat were not marked with <br /> the date it was emptied/not managed w/in one year of being empty. All drums bearing <br /> labels from prior use have been removed from the site. None of the drums which <br /> have been removed were used to contain Hazardous Waste or Hazardous Materials. <br /> All remaining drums used to contain Hazardous Waste and Hazardous Materials <br /> have been labeled properly and will be disposed of within 90 days of label date. <br /> 89. An empty HW or Haz Mat container (larger than 5 gallons) was not marked with the <br /> date it was emptied/not managed within one year of being emptied. This container has <br /> been properly disposed of by American Valley Waste Oil.
