Manifest numbers 003593183FLE (shipped 04/07/11) and 008159461 JJK (shipped <br /> 07/08/11) were found without a signed off copy from the destination facility. Hazardous waste <br /> generators shall retain copies of all manifests signed off by the destination facility on site for three years <br /> and have them readily available for review. If the generator did not receive a copy of the manifest with <br /> the handwritten signature of the owner or operator of the facility to which the generator's waste was <br /> submitted within 35 days of the date the waste was accepted by the initial transporter, the generator <br /> shall contact the transporter and/or the owner or operator of the designated facility to determine the <br /> status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within <br /> 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an <br /> Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the <br /> generator explaining the efforts to locate the hazardous waste and the results of those efforts. <br /> Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. <br /> Submit to the EHD a copy of the signed off copy of the manifest or a copy of the exception report <br /> submitted to DTSC. <br /> Submit to SJC EHD within 30 days (08/16/11)- <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for the above violation <br /> 3. requested/supporting documentation <br /> Notes: <br /> - The manifest folder divided into 3 sections: Generated (off site during work on the hwy), Accidents <br /> (during Emergency Response), Generated Shop 10 (waste generated from Equipment Bldg/ Mechanic <br /> Repair shop on site). Generated and Generated Shop 10 utilizes same EPA ID CAD982497828. <br /> Accidents utilizes EPA ID CAS111111039 <br /> - In 2010 year, facility generated approximately 30.6 Tons of manifested hazardous waste and 380 <br /> gallons (1.52 Tons) of used oil = 32.12 tons of hazardous waste under EPA ID CAD982497828. <br /> Generated Shop 10 waste is estimated <15 tons. <br /> - Provided free training class schedule for July - December 2011 year.
