Personnel training records incomplete. At least three employees who handle hazardous waste have <br /> not had their annual refresher training in CalTrans Hazardous Substance First Responder. Facility <br /> personnel shall complete a training program in accordance with Title 22 regulations. This program shall <br /> be directed by a person trained in hazardous waste management procedures and shall include the <br /> following- <br /> - hazardous waste management procedures, including contingency plan implementation <br /> - emergency procedures, equipment, and systems, <br /> - key parameters for automatic waste feed cut-off systems <br /> - communications or alarm systems - shut down of operations <br /> - response to fire, explosion, or groundwater contamination incident <br /> Immediately prepare and implement a complete hazardous waste management training program for this <br /> facility in accordance with Title 22 regulations and ensure that all employees complete this training at <br /> least annually. Submit a statement how to ensure all employees who handle hazardous waste will <br /> receive their annual refresher. <br /> 58. Manifest has incorrect generator's address. All the uniform hazardous waste manifest generated by <br /> hazardous waste hauler Safety-Kleen System Inc lists the generator's site address as Cal Trans Dist 10, <br /> Equipment Division, 24155 S. Corral Hollow, Tracy, CA, 95376. The waste removed by Safety-Kleen is <br /> from the aqueous waste from the parts washer that is located in Equipment Bldg/Mechanic repair shop. <br /> Generator of hazardous or extremely hazardous waste shall ensure the generator and waste sections <br /> are written correctly and sign the manifest certification.
