Spill container testing was performed on 9/ 19/ 17 and a test report was submitted on 3/23/ 18 , 6 months late . A copy of the test report must be submitted within 30 days of the tests . Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. The operator provided a statement to close this violations while the inspector was on site . This was corrected on site . This is a repeat violation , Class ll . 201 CCR 2712 ( b ) Failure to maintain monitoring , maintenance , and follow up action records . Maintenance and monitoring records for the last three years were not found on site . An alarm for L- 13 UDC 7/8 occurred on 5/4/ 18 , however no records on how this alarm was responded to were found on site . These records shall be maintained on site for at least three years . Monitoring records include : ( 1 ) date and time of all monitoring or sampling ; (2 ) monitoring equipment calibration and maintenance records ; (3 ) results of any visual observations ; (4 ) results of sample analysis performed a lab or in the field ; ( 5 ) logs of all readings of gauges or other monitoring equipment , ground water elevations , or other test results ; (6 ) results of inventory readings and reconciliations . Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD . The operator provided a statement to close this violations while the inspector was on site . This was corrected on site . This is a repeat violation , Class II , 210 CCR 2716 ( f) DO monthly reports not maintained on site for 12 months or for 36 months ( after October 1 , 2018 ) . The June 2018 , August 2018 and September 2018 designated operator monthly inspection reports were not found on site . Designated operator monthly inspection reports for the previous twelve months shall be retained on site . Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site . Submit copies to the EHD .
