Time In: 11:05 am <br /> Time Out: 12:00 i3m <br /> Food Program Complaint Inspection Report <br /> Name of Facility: Date: 02/20/2019 <br /> Address: 3434 MANTHEY RD, STOCKTON <br /> Owner/Operator: JENKINS ENTERPRISES LLC Telephone: <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00048794 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> C00048794 states: "RATS IN THE FACILITY, INFORMED MANAGEMENT WHICH SAID THERE ISN'T ANYTHING THEY <br /> CAN DO ABOUT IT." <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Store Director(SD)is aware of the complaint. SD states that there is one rat in the facility. It has been here for weeks and still <br /> has not been caught(SD and exterminator observed rodent this date during the exterminator's visit). Facility utilizes Western <br /> Exterminators. For the past 3 weeks,Western has been making 3 visits per week.The area of the store affected is the south <br /> side. <br /> Corrective action taken by facility: numerous glue boards and snap traps(observed 20+this date)have been placed <br /> throughout the facility under shelving and end caps, external bait boxes are in place, extensive cleaning of the store has been <br /> made within the past week(all bottom boards of shelves have been removed and cleaned under at south side of the store- <br /> observed photos on SD's phone of cleaning progress). end caps have been moved and cleaned,the pallets in the rear <br /> warehouse receiving area have been moved out and cleaned under, and the bread and tortilla areas have been cleaned <br /> today. <br /> Observations this date: a bag of rice was sliced open. Bag of rice was removed and food debris will be cleaned up. <br /> Corrective action required: 1) Provide exterminator invoices for the past two months. E-mail to Kadeanne Linhares <br /> (klinhares@sjcehd.com)2) Monitor areas of concern daily for any new evidence of activity(droppings,gnaw marks, rub <br /> marks). If food items or paper/disposable items are observed to have gnaw marks, discard affected items).
