(113996, 113998, <br /> 114037, 114343(a)) <br /> #23 Rodents, Insects or Animals Inside Facility MAJOR <br /> OBSERVATIONS:Observed fresh and dry rodent droppings in the following locations: in the mop sink, on top of the dish <br /> machine, on the floor behind the mixer, on the shelving underneath the dough sheeter, on top of the pizza oven, behind the <br /> pizza prep, on the floor under the rack to the right of the pizza oven, and on top of boxes in the dry goods storage room. <br /> Observed hair(rodent hair?)and possible rub marks on top of the dish machine. <br /> Observed gnaw marks/chewed on foam insulation on the floor underneath the dish machine. <br /> ***No rodent droppings observed on food contact surfaces this date." <br /> Immediately clean up all rodent droppings, hair, and chewed foam throughout the facility. Clean and sanitize the affected <br /> areas. Closely inspect all areas for potential entry points.
