During the January 29, 2018 inspection, water was found in the diesel, <br /> 87, and 91 submersible turbine pump (STP) sumps, 87 and diesel fill sumps, vent box sump, and under dispenser <br /> containment (UDC) sump in dispenser#3/4. The received return to compliance documentation did not adequately <br /> address this violation. This information was required to be submitted by March 1, 2018. The adequate return to <br /> compliance information has not been received resulting in a non-compliant status for this facility. An operator that <br /> receives an inspection report shall have 30 days to submit a written response that includes a statement <br /> documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written <br /> response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection <br /> report, submit proof of correction to the EHD. <br /> This is a Class II violation. <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator failed to document all the alarms from the attached alarm history on the April 26, 2018 <br /> designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br /> missing alarm includes: Q3 alarm which occurred on April 6, 2018. During the monthly inspection, the designated <br /> operator shall review the alarm history for the previous month, check that each alarm was documented and <br /> responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br /> including all of the required information on the reports. <br /> This is a Class II violation. <br /> 305 HSC 25290.1(e)VPH monitoring of the interstitial spaces of the UST system is not maintained. <br /> The S10 smart sensor located on the vapor line in the 87 STP did not go out of alarm after it was tested. The <br /> continuous monitoring system shall be maintained to detect the entry of the liquid or vapor phase of the hazardous <br /> substance stored in the primary containment into the secondary containment and to detect water intrusion into the <br /> secondary containment. The sensor was replaced and retested during the inspection. A"UST Retrofit Verification <br /> with Inspector Already Onsite"form has been completed and provided to the operator and contractor. This was <br /> corrected at the time of inspection. <br /> This is a repeat violation, Class II. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 91 submersible turbine sump (STP)sensor was positioned in a sensor holder located approximately 3-4 inches <br /> up the sump saddle and off the lowest point of the sump and not located to detect a leak at the earliest opportunity. <br /> Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. <br /> Immediately relocate this sensor to the lowest point of the 91 STP sump, submit proof or correction to the EHD. <br /> This is a Class II violation. <br /> FA0003635 PR0231656 SCO01 01/22/2019 <br /> EHD 23-01 Rev.09/28/2018 Page 4 of 5 Underground Storage Tank Program OIR <br /> 1868 E.
