SKJ OAQU I N Environmental Health Department <br /> COUNTY- AMENDED <br /> t-��tf-t <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ARCO 06080 85 E LOUISE AVE, LATHROP January 22 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item # Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator failed to document all the alarms from the attached alarm history on the April 26, 2018 <br /> designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br /> missing alarm includes: Q3 alarm which occurred on April 6, 2018. During the monthly inspection, the designated <br /> operator shall review the alarm history for the previous month, check that each alarm was documented and responded <br /> to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the <br /> monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the <br /> required information on the reports. <br /> This is a Class II violation. <br /> 305 HSC 25290.1(e) VPH monitoring of the interstitial spaces of the UST system is not maintained. <br /> The S10 smart sensor located on the vapor line in the 87 STP did not go out of alarm after it was tested. The <br /> continuous monitoring system shall be maintained to detect the entry of the liquid or vapor phase of the hazardous <br /> substance stored in the primary containment into the secondary containment and to detect water intrusion into the <br /> secondary containment. The sensor was replaced and retested during the inspection. A"UST Retrofit Verification <br /> with Inspector Already Onsite" form has been completed and provided to the operator and contractor. This was <br /> corrected at the time of inspection. <br /> This is a repeat violation, Class ll. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 91 submersible turbine sump (STP) sensor was positioned in a sensor holder located approximately 34 inches up <br /> the sump saddle and off the lowest point of the sump and not located to detect a leak at the earliest opportunity. <br /> Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. Immediately <br /> relocate this sensor to the lowest point of the 91 STP sump, submit proof or correction to the EHD. <br /> This is a Class II violation. <br /> 308 CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br /> The 87 STP liquid sensor failed when tested. The continuous monitoring system shall be maintained to detect the <br /> entry of the liquid or vapor phase of the hazardous substance stored in the primary containment into the secondary <br /> containment and to detect water intrusion into the secondary containment.
