At 1635 1 left the incident scene. <br /> Refer to Elena narratives for additional information about this incident. <br /> 09/01/2016 Received call from Haza Saeed that a a truck released diesel over train tracks in PE 2546 ZAREEF <br /> 18 min Tracy on Wicklund Rd. Haza said he received the call from Public Works (Troy SC 977 <br /> Hedrick) who received it from the (Eric Gagty) CHP. Called the CHP and spoke to <br /> Ardele who is a Public Safety Dispatcher. She said a truck crossed the tracks and <br /> ruptured the fuel tank. No one was injured the CHP has the tracks closed down. It <br /> happened at 6:16 AM and the fire department was on the scene. Called Kasey to <br /> discuss. It will be handed over to the C person. <br /> 09/01/2016 1 arrived at my house at 1733 (Travel Time) PE 2546 SAEED <br /> 30 min <br /> SC 944 <br /> 09/01/2016 Discussed the spill with Kasey Foley. 9:30 pm onsite. The truck trailer is still PE 2546 MANZO <br /> 210 min parked at the site of the incident, the truck is parked to the side(license plate SC 004 <br /> L653619, out of Quebec Canada).The leak extends from the rail way crossing <br /> along the Wicklund and until Wicklund meets Byron. Haza is onsite as well. <br /> Absorbent was applied. The RP is getting the cleanup company retained. Since <br /> the leak is on the train tracks, called the UP critical call center(RNCC).Spoke with <br /> Dave, who transferred me to Robert Bavier(951-323-2366)the hazardous <br /> material's specialist in charge of the area. The UP was not aware of any leak on <br /> their line, provided spill details and contact information to the responsible party for <br /> UP to coordinate clean up. 10:58 am, received a call from Dave with UP who was <br /> making notifications and wanted to know if I was with CUPA. Issued a notice to <br /> abate for the clean up, provided a copy to the responsible party (Francine onsite), <br /> discussed with Francine the hazardous waste manifesting procedures. Started <br /> Prop 65 and ER record. 11:30 am left the site to go to another spill. 1:30 pm, <br /> returned to the site and observed the clean up. Provided clean up update to Kasey <br /> Foley(program coordinator) along with asphalt pictures showing the deteriorated <br /> asphalt condition. Kasey indicated that we will not be requiring asphalt <br /> replacement or repair. Left the site to go to another spill, Haza remained to <br /> witness the final clean up. <br /> 09/02/2016 Prop 65 preparation. PE 2546 MANZO <br /> 24 min <br /> SC 203 <br /> 09/02/2016 1 completed the Narrative for this incident. I filled out overtime spill, printed the PE 2546 SAEED <br /> so min daily, and submitted to Kasey. SC 007 <br /> 09/02/2016 Received a phone call from Francine (RP) driver involved in the incident, PE 2546 MANZO <br /> 30 min discussed the manifest, told her that i will need the final signed manifest copy, SC 544 <br /> discussed disposal process, the indicated that she will provide the final copy when <br /> they received it. Documented the spill response. <br /> 09/19/2016 1 completed the incident report including compliant investigation form, incident PE 2546 SAEED <br /> 48 min photos, incident map and emergency response record. I attached the spill report, SC 544 <br /> notice to abate, and Prop 65 . I Submitted for review. <br /> 07/06/2017 Printed HWTS info showing that manifest 014654786JJK was received by the PE 2546 BACKUS <br /> 12 min TSDF on 9-23-17. SC 007 <br /> Page 3 of 3 <br />
