Please note the report was received in the SJCIEHD office on November 5, 2003.
The report documents a routine quarterly groundwater monitoring and sampling event performed
on June 10, 2003. In Section 7.0 of the report, JJW states that a work plan has been submitted to
SJCIEHD, and that pending approval of the work plan, remediation of the site should begin.
SJCIEHD notified you in correspondence dated October 1, 2003, that the work plan JJW is
referring to was conditionally approved. Please refer to that dated correspondence for details of
the conditions to the work plan. The deadline for the approved work to be completed is
December 29, 2003. Furthermore, you were directed in the same correspondence that a work plan
to address the remediation of the vadose zone soil contamination was due for submittal to
SJCIEHD by November 24, 2003.
The Report contains another recommendation to destroy damaged monitoring well MW-3. The
destruction of this well was approved by SJCIEHD in correspondence dated October 31, 2002.
JJW has stated the well will be destroyed by over drilling to 45 feet below surface grade and
backfilling with neat cement grout. Please submit a SJCIEHD well destruction permit application
and required $60.00 permit fee, and schedule the approved work.
Laboratory analytical results for this site are not being electronically submitted to GeoTracker, the
State Water Resources Control Board database program. This is required by Water Code Section
13196, California Code of Regulations Section 2729.1 and is commonly referred to as AB 2886.
Electronic transmission of laboratory analytical results to GeoTracker must be initiated immediately.
Donna Heran, REHS, Director
Environmental Health Department
Lori Duncan, Senior REHS Margaret L gorio, Supervising REHS
LOP/Site Mitigation Unit IV LOP/Site Mitigation Unit IV
cc: James Barton, CVRWQCB
Mark Jerpbak, JJW
