Page 5
May 5, 2014
Project Number 2014-03-005
Health and Safety
At least 48 hours prior to any digging at the site, Dig Alert was contacted so that the utilities could
be marked prior to start of work. Prior to start of work, a brief "tailgate safety meeting" was
conducted to inform the field crew of anticipated hazards and the emergency action plan for the
site.
Sample Locations and Collection Methods
The sampling event occurred on April 10, 2014. Four boreholes were advanced onsite, including
one at the inlet and one at the outlet of the oil/water separator and one at each side of the oil/water
separator. A fifth borehole was hand augered and abandoned at 1 foot due to an obstruction at one
foot below the ground surface. Each Borehole was hand augered to 4 feet prior to drilling. The
boreholes were advanced by a Geoprobe truck-mounted hydropunch rig unti I total depth of 20 feet
below the ground surface. In each borehole, soil samples were collected by the environmental
professional. The samples were screened by a PID and select samples were collected from the
desired strata and placed into laboratory provided containers, capped, and labeled. All samples
were observed for contamination. The selected samples were immediately placed on ice and
transported under chain of custody to Test America Laboratories in Phoenix.
The samples were analyzed for the following constituents of concern:
Soil Sample ID Anal
B-1 TPH gas, TPH diesel, TPH motor oil,VOCs by
8260B, arsenic, barium, cadmium, chromium, lead,
mercury, selenium, and silver.
©ENVIRO ASSESS ENVIRONMENTAL•ENGINEERING•GEOLOGY
PO BOX 1154,BONNERS FERRY,ID 83805 PHONE(877)629-6838 FAX(877)623-5493
