In their correspondence dated March <br /> 16,2010,the SJCEHD requested destruction of the site-related groundwater monitoring wells. <br /> i <br /> 1.3' Project Objectives <br /> Groundwater monitoring wells MW-9, MW-1l through MW-13,MW-14A, MW-16;MW719,MW-20, <br /> MW-24, MW-25, VM-1, VM-2, ASWA through ASW-4, and VEW-1 and VEW-2 will be destroyed <br /> per SJCEHD requirements..-" <br /> . o <br /> 1.4 Planned Scope of Services <br /> • General survey/non-intrusive activities <br /> • Well Destruction activities <br /> 1.5 Schedule <br /> Anticipated Period of Performance: July 2010 <br /> Anticipated Weather/Temperature: Weather conditions are expected to be warm to hot <br /> 2.0 ADMINISTRATIVE REQUIREMENTS/CONTROLS <br /> 2.1 Personnel <br /> Personnel responsible for project safety include the Project Manager,the Site Safety Officer(SSO),the <br /> Consulting Certified Industrial Hygienist(CIH),and participating project personnel.' <br /> 2.1.1 Proiect Manager <br /> The Project Manager has ultimate authority and responsibility for project Health and Safety. <br /> Accordingly, he/she has the.responsibility to develop the HSP (or. assign its development); audit <br /> compliance with the provisions of this HSP; suspend project activities or.modify service practices for <br /> health and safety reasons; and, to dismiss from a project site individuals whose onsite conduct either <br /> endangers the health and/or safety of others or is judged not to comply with the provisions of the HSP. <br /> The Project Manager is responsible for sharing/distributing the HSP to participating field personnel and <br /> to an authorized representative of each project subcontractor. The Project Manager is also responsible <br /> for implementing all provisions of the HSP and any applicable addeiida. Implementation includes: <br /> • Reviewing the HSP requirements(if prepared by another project member); <br /> Shop 10 MS,Task Order No.94 Caltrans Contract No.06A 114.1-,EA 43-910076 <br /> Project No. 59200-06-94 -27 July 2010 <br />
