30 SCOPE OF SERVICES <br /> Geocon performed the following scope of services, in accordance with Caltrans TO No 10-OE7900- <br />' MG, which included site meetings, fieldwork, chemical analyses of soil and groundwater samples, <br /> and preparation of this report <br />' L11 Pre-]Field Activities <br /> o A pre-work site visit and TO meeting were conducted on October 19, 2001, to observe the work <br /> I area, review project drawings, and mark proposed boring locations for subsequent underground <br /> utility clearance Caltrans representative Mr Saiyed All and Geocon representatives Mr John <br /> Mattey and Mr Robert Kimball attended the pre-work meeting Following the meeting, the Site <br />' Visit Checklist, Completion Schedule, and Notice to Proceed were discussed and signed <br /> 0 Geocon prepared the Site Investigation Workplan dated November 6, 2001 The Workplan <br /> describes the purpose of the scope of work, proposed boring locations and field methodologies, <br />' and the laboratory analytical program for the project <br /> o Geocon prepared a Health and Safety Plan (HSP) for the Site dated October 31, 2001 The HSP <br /> provides guidelines on the use of personal protective equipment and the health and safety <br />' procedures implemented during the field activities <br /> o Geocon obtained well destruction permit (Permit No 28093), monitoring well permit (Permit <br />' No 28092), and CPT boring permit (Permit No 28102), and pard applicable fees to the San <br /> Joaquin County Environmental Health Department (SJCEHD) Copies of the permits are <br /> presented in Appendix A <br /> o Geocon obtained a Caltrans encroachment permit (Permit No 1001-NMC-1005) dated <br /> November 8, 2001, prior to drilling activities <br /> o Geocon provided at least 48-hour notification to the Underground Service Alert prior to job site <br />' mobilization(USA Ticket No 354470) <br /> o Geocon retained the services of Gregg Drilling, Inc , a California C57-licensed and Caltrans- <br /> approved subcontractor located in Martinez, California,to perform the CPT borings at the Site <br /> o Geocon retained the services of V & W Drilling, Inc , a California C57-licensed and Caltrans- <br /> approved subcontractor located in Isleton, California, to perform the monitoring well destruction <br />' and construction activities <br /> o Geocon retained the services of Cruz Brothers, a Caltrans-approved subcontractor located in <br />' Scotts Valley, California,to provide additional underground utility location at the Site <br /> o Geocon retained the services of Zymax Envirotechnology to perforin chemical analyses of soil <br /> and groundwater samples Zymax is a Califorma-certified and a Caltrans-approved analytical <br />' laboratory located in San Luis Obispo, California <br /> o Geocon retained the services of Morrow Surveying, a California-licensed and Caltrans-approved <br /> surveyor to survey the top of casing elevation of the monitoring well <br /> Project No 58475-06-02 -3- December 26,2001 <br />
