PHS/EHD has
prepared a summary, for your information, of the agreements made by Subhash Johar on February 6,
1995 regarding this site.
PHS/EHD was informed that the monitoring well caps were replaced with expandable locking caps, as
recommended by your consultant, and that the wells will be sampled by March 1, 1995 with the
subsequent first quarter 1995 monitoring report to be submitted by April 15, 1995. Also, in order,to
comply with the PHS/EHD directive to investigate the extent of groundwater and soil contamination, a
task order, to obtain funding for a work plan and its implementation, will be prepared and submitted
for internal Caltrans approval. The work plan to perform the required groundwater and soil
investigation will be submitted to PHS/EHD by May 15, 1995.
Please be aware that should you be unable to meet the due dates, a request for extension should be
submitted prior to the due date. If you have any questions or comments, please contact me at (209)
468-0337.
I
Donna Heran, REHS, Director
Environmental Health Division
Mary Meays, Senior REHS Diane M.
