The training records for 2011 did not include a signature for each employee. No records were <br />available for additional trainings. The hazardous waste training program shall be completed by each <br />employee within six months of hire, prior to unsupervised work, and shall be reviewed at least annually. <br />Immediately provide current training as necessary for each employee and ensure that employees receive <br />this training annually. Submit a copy of the updated training records to the EHD by May 25, 2012. <br />117. The training records on site did not include written job descriptions. Training records shall be <br />maintained on site until closure of the facility and shall include: <br />- name & job title of each employee <br />- written job description for each job title, including requisite skill, education, qualifications, and duties <br />- description of training to be given to each job title initially and as an annual refresher <br />- records that training was completed for each employee <br />Submit a copy of the updated training records to the EHD by May 25, 2012. <br />132. No inspection logs were available for the used oil and used antifreeze tanks. The owner or operator <br />shall inspect, at least once each operating day, all of the following: <br />(1) overfill/spill control equipment (e.g., waste -feed cutoff systems, bypass systems, drainage <br />systems) to ensure it is in good working order <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE <br />
