(continued) Immediately prepare and implement a complete hazardous waste management training program for this facility in accordance with Title 22 regulations and ensure that all employees complete this training at least annually. Submit a copy of the training program and records to the EHD by March 20, 2011. REPEAT VIOLATION 60, 61, & 62. Manifest number 003924979FLE (11/24/10) was found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. By March 20, 2011, submit to the EHD a copy of the signed off copy of the manifest or a copy of the exception report submitted to DTSC. REPEAT VIOLATION Notice to Comply - Minor violations: (21, 31, 85) 21. A 55 gallon black steel drum of used drained oil and fuel filters was observed labeled as "hot drained oil filters." Containers of commingled filters shall be stored in containers that are designed to prevent ignition of the gasoline and labeled "used oil and gasoline filters." Immediately provide proper labeling for this container and ensure that all commingled filter containers are marked with all the required information. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
