- 3 - 1 September 2006 <br /> New West Station No. 100 <br /> PUBLIC NOTIFICATION <br /> Pursuant to Section D of Attachment A of General Order No. 5-00-119, a Public Notice was <br /> sent to all recorded landowners within a 500-foot radius of the site and proposed discharge <br /> location. The notice, dated 8 December 2005, requests that written or verbal comments be <br /> received by our Regional Water Board office within 14 days from the date of the Public Notice. <br /> No public comments have been received in our office for this project. <br /> GENERAL INFORMATION <br /> 1. Project activities shall be operated in accordance with requirements contained in the <br /> General Order and in accordance with the information submitted in the RWD. <br /> 2. The Discharger shall notify Regional Water Board staff of the specific time schedule <br /> for project implementation and of major changes in operation, three business days <br /> prior to commencement of work. The Discharger shall notify Regional Water Board <br /> staff at (916) 464-3291. <br /> 3. The Discharger shall notify Regional Water Board staff within 24 hours if any <br /> nuisance or detriment to the receiving water is observed during the implementation <br /> of the project. The Discharger shall notify Regional Water Board staff by telephone <br /> at (916) 464-3291 and send a letter describing the observations to the following <br /> address: <br /> Central Valley Regional Water Board <br /> 11020 Sun Center Drive, #200 <br /> Rancho Cordova, CA 95670 <br /> 4. The Discharger shall notify Regional Water Board staff within 24 hours of problems <br /> and/or difficulties encountered during implementation of the project. The Discharger <br /> shall notify Regional Water Board staff by telephone at (916) 464-3291 and send a <br /> letter describing the problems/difficulties, and the actions taken to remediate the <br /> matter, to the address shown in Item No. 3 above. <br /> 5. The required annual fee (as specified in the annual billing you will receive from the <br /> State `,Nater Resources Control Board) shall be submitted until this Notice of <br /> Applicability is officially terminated. Please notify this office in writing, requesting the <br /> termination of the Notice of Applicability, when the project has been completed and <br /> the discharge is no longer necessary. As required by the Monitoring and Reporting <br /> Program, a post-discharge report shall be completed and submitted to the Regional <br /> Water Board. The report may be included with the notice of termination. <br /> 6. General Order No. 5-00-119 (enclosed) prescribes minimum wastewater monitoring <br /> and reporting requirements for compliance. <br /> 7. The initial treated groundwater shall be contained in a holding tank until laboratory <br /> results confirm compliance with the effluent requirements of General Order <br /> No. 5-00-119. <br />
