Scott Mansholt <br /> Chevron Environmental Management Company <br /> ' 6111 Bollinger Canyon Road <br /> San Ramon, California 94583 <br /> ' Subject: Groundwater Monitoring Well Destruction Report <br /> Former Lyoth Loading Station <br /> ' 26501 Banta Road <br /> Lyoth, San Joaquin County, California <br /> Dear Mr. Mansholt, <br /> Science Applications International Corporation (SAIC) prepared this letter at [he request <br /> of Chevron Environmental Management Company (CEMC) to document the destruction <br /> of groundwater monitoring wells MW_1, MW-2, MW- 3, MW-4, MW-6, and MW-7 at <br /> the former Lyoth Loading Station located at 26501 Banta Road in Lyoth, California (see <br /> Figures 1 and 2). The California Regional Water Quality Control Board (RWQCB) <br /> ' Central Valley Region accepted the well destruction work plan in a June 2009 letter to <br /> CEMC' (see Attachment A). <br /> WELL DESTRUCTION ACTIVITIES <br /> SAIC obtained permits to destroy the wells from the San Joaquin County Department of <br /> Environmental Health (SJCDEH) on August 6, 2009 (see Attachment A). Underground <br /> utility clearance was requested through Underground Service Alert on October 21, 2009. <br /> Cruz Brothers Locators, a private utility locator, was subcontracted to clear any potential <br /> ' overhead and underground hazards. Site access was obtained through pre-existing access <br /> agreements dated May 4, 1994 (for the former Lyoth Loading Station property), and <br /> March 7, 2003 (latest extension date for access to the railroad property). <br /> ' Wells MW-1, MW-2, MW-3, MW-4 MW-6, and MW-7 were constructed with 2 inch- <br /> diameter polyvinyl chloride (PVC) well casing. Well construction details were not <br /> ' available; therefore, the bottoms of casing depths were measured prior to commencement <br /> of overdrilling activities.
