' Mr. M. Scott Mansholt—CEMC
Groundwater Monitoring Well Destruction Report November 17, 2009
Former Lyoth Loading Station
23.15 and 31.7 feet below ground surface (bgs). All of the well heads were completed
with a stovepipe-type well monument surrounded by three traffic bollards.
' Well destruction activities were conducted from October 26 through October 28, 2009.
All well destruction activities were managed by on-site SAIC field personnel under the
' supervision of an SAIC California Professional Geologist. RSI Drilling (C-57 licensed
driller) performed the well destruction. SJCDEH was notified of the well destruction
activities prior to the commencement of field activities.
The condition of each well was inspected prior to destruction. The total depth of each
well was measured and confirmed with respect to the assumed well construction details.
Each well location and surrounding traffic bollards were cleared for utilities by advancing
t three perimeter hand-auger borings; these borings were advanced to 1 foot bgs for the
bollards and 8 feet bgs for the wells.
' RSI Drilling removed traffic bollards and wellheads prior to overdrilling the wells. A
CME75 truck-mounted drill rig fitted with 9.25 inch-diameter augers was used for
overdrilling the sand pack and sealant to the total depth of each well. The boreholes were
' backfilled via trernie pipe by neat cement to the ground surface.
All wastes—including concrete, well monuments, and PVC well-casing material—were
stored on site pending disposal by Sturgeon & Son, Inc.
' Copies of the Well Completion Reports, as submitted to the California Department of
Water Resources (DWR), are included as Attachment B.
' If you have any questions regarding this work, please contact me at (510)466-7161.
Sincerely,
' SCIENCE APPLICATIONS INTERNATIONAL CORPORATION
Michael Hurd, CHG 0068
Senior Environmental Project Manager y` '''`` UT,JT
Enclosures: %
' Figure 1.
