work plan, such as ADDING, moving, or removing "proposed boring' locations as shown on the
figure in the WP.
From you phone call just a few minutes ago, I got the impression there have been changes to
what currently appears on FIGURE 5 of the work plan. IF so, send me the new map showing the
changed boring locations.
IF no changes, then I can process the applications assuming the money concern below is
addressed.
Unfortunately you sent only ONE check for ALL the applications. Typically a separate check is to
accompany each application. But if your math is correct and all the necessary submittals are on
my desk and I can process them all at_one_time,this works just fine with one check. But IF, as in
this case, some applications are NOT going to be processed (WP 3-29-06 across the street)then
the money you included in the ONE check gets either: OPTION 1- RETURN the check to you so
you can re-send us (two) separate $89 checks, one per application OR, OPTION 2, the
remainder/balance of the check amount is added to your account to cover inspection and office
charges. I have check SAIC check#0001686677 ($457.00). After processing these two
applications at $89 each, the balance will be $279.00.When it comes to processing the other
applications, then an $89 check per permit application will be needed before I can process.
LET ME KNOW WHICH OPTION YOU WOULD PREFER TO DO.
In order to complete the processing of these applications TODAY, I need
you to answer/address the following items:
1- if boring locations/number have changed from FIGURE 5 in the WP, then send me anew
map and,
2- return the one large check to you OR process the two permits and put the $279 balance
in your account.
I have other appointments later this am and ALL afternoon. Lets get this handled in the next hour
or so please.
From: Slowik, William P [mailto:WILLIAM.P.SLOWIK@saic.com]
Sent: Wednesday, January 03, 2007 4:32 PM
To: Mike Infurna [EH]
Cc: Dennis, Christopher B; Anzelon, Daniel B.
Subject: RE: Chevron EMC's Lyoth Site
Mike,
Please refer to the March 231d work plan for Lyoth Offsite. We will only be advancing the
borings which appear on Figure 5 in the March 23rd Lyoth Offsite work plan. There are a
total of four borings —three confirmed borings and one contingency boring—associated
with this work plan.
One of the confirmed borings is located on SJC assessor parcel 252-07-004, a property
which is operated as Reed and Sons Towing. As you may recall, the property owner
identified on the Site Access Agreement in SAIC's original permit application submittal
was not up to date (he had since sold the property unbeknownst to us). We recently (late
November/early December)submitted to you an updated Site Access Agreement
reflecting the current property owner.
The other two confirmed borings and the one contingency boring are located on Union
Pacific Rail Road property (SJC assessor's parcel 252-01-003)and you have the
complete and correct permit application package for this parcel. If you give us the green
light, next Monday, January 8th,we'll be working on the UPRR property first and then the
Reed and Sons Towing parcel.
FYI, the March 29th work plan if for a site called Banta Road—UPRR which is right
across the street. We have not submitted any permit applications for Banta Road —
UPRR because we do not have all of our access agreements in order. Therefore,we will
1/4/2007
