I also want to assure you that there is no hurry for approval.
Thanks again,
Daniel Anzelon
From: MInfurna@sjcehd.com [mailto:MInfurna@sjcehd.com]
Sent: Wednesday, October 25, 2006 8:39 AM
To: Anzelon, Daniel B.
Subject: RE: Site Mitigation Fee
I'm back in the office today, Oct 25.
Just in case you haven't handled this already, here's some info for you.
Our accounting office has indicated that this site, LYOTH Loading is in good standing and the billing
responsible party is Chevron Env Mgmt.
If CEM is still the correct billing party and will pay our inspection fees when billed, then NO prepayment is
needed at this time. IF CEM is NOT the billing RP, then YES, submit the NEW fee of$285 along with the
MFR form.
Permit application fee is still $89 per parcel (installations). If parcel NOT owned by CEM, then a signed
access agreement is needed before EHD can issue the permit.
You can call me at(209)468-3454 if you have questions. I have a very busy drill schedule today starting
about 930a and I'll be gone most of the day.
10/26/2006
