REC EKED <br /> October 18, 2006 <br /> OCT 2 0 2006 <br /> ENVIRONMENT HEALTH <br /> PERMIT/SERVICES <br /> San Joaquin County <br /> Environmental Health Department <br /> 304 East Weber Avenue, 3'd Floor <br /> Stockton, CA 95202-2708 <br /> Subject: Well Permit Application for Soil Borings <br /> Lyoth Offsite <br /> Lyoth, CA <br /> Dear San Joaquin County Environmental Health Department, <br /> At the request of Chevron Environmental Management Company(Chevron), contractor <br /> Science Applications International Corporation(SAIC)has prepared this well permit <br /> application for the installation of three soil borings. The 2 inch diameter borings will be <br /> advanced approximately 30 feet deep. The borings will be back filled with neat cement <br /> after collection of soil and groundwater samples. <br /> The driller for this project is RSI Drilling, C57# 802-334. SAIC requests that a copy of <br /> the approved permits be sent to Daniel B. Anzelon, SAIC,3800 Watt Ave, Suite 210, <br /> Sacramento, CA 95821. <br /> SAIC has not yet scheduled the work; however we anticipated the job to take place in <br /> mid-November. SAIC will notify The San Joaquin County Environmental Health <br /> Department at least 48-hours prior to beginning field work.
