Mr.M. Scott Mansholt—CA •
Request to Properly Destroy Monitoring Wells May 26,2009
Former Lyoth Loading Station,Lyoth,CA Page 2 of 2
the depth of the proposed well destruction and the auger diameter. Actual well depths
will be confirmed in the field.
SAIC will coordinate the proper destruction of wells MW-1, MW-2, MW-3, MW-4,
MW-6, and MW-7 by:
• Obtaining the appropriate well destruction permits from the San Joaquin County
Environmental Health Department(SJCEHD).
• Notifying Underground Service Alert at least 48 hours in advance of field activities.
• Contracting an underground utility locator to clear subsurface utilities in the area of
the wells to be destroyed.
In accordance with SJCEHD guidelines, the well casing and filter pack will be over
drilled to the total depth of the well, using a truck-mounted drill rig equipped with
8.25 inch-diameter hollow-stem augers as indicated in Table 1. Well destruction
activities will be managed by an on-site SAIC field geologist under the supervision of an
SAIC California Professional Geologist. The well boring will be backfilled with neat
cement to surface grade using a tremie pipe. The existing bollards will be removed, and
any depression left by removal of the bollards will be backfilled to surface grade with
neat cement. Well construction details were only available for MW-7 and are provided
as Attachment B. SAIC will notify SJCEHD and the RWQCB of the proposed well
destruction a minimum of 48 hours in advance.
On completion of the well destruction, SAIC will prepare a report documenting the
destruction activity and submit the report to the SJCEHD and the RWQCB. SAIC will
submit a well drillers report (Form DWR 188) to the California Department of Water
Resources as required.
If you have any questions regarding this request, please call Michael Hurd at (510) 466-
7161.
Sincerely,
SCIENCE APPLICATIONS INTERNATIONAL CORPORATION
Michael Hurd, CHG 0068
Senior Environmental Project Manager
Enclosures: s'
Figure 1.
