Additionally, you <br /> were directed to submit Quarterly Monitoring Reports by the fifteenth (15th) of the month <br /> following the quarter's end. The work plan and Quarterly Monitoring Report for the First <br /> Quarter-2001 have not been received. <br /> As of the date of this letter, none of the directive's submittal requirements have been met. <br /> Therefore, you are not in compliance with directives issued by PHS-EHD. The work plan <br /> (complete with all forms) and the First Quarter-2001 Monitoring Report are past due and to be <br /> submitted immediately. <br /> This is your official 90-day notice as required by the State Water Resources Control <br /> Board Underground Storage Tank Clean Up Fund (Fund) that you have failed to maintain <br /> the minimum requirements to become or remain eligible to receive reimbursement for <br /> your required investigation. <br /> If you have not complied with these directives by June 25, 2001, you may be removed from the <br /> priority list and you may be disqualified from the eligibility list. However, when all of directives <br /> have been successfully met, PHS-EHD will declare that you are in compliance and your status <br /> with the Fund will no longer be compromised. <br /> As a reminder, the February 27, 2001 directive letter also requires the Problem Assessment <br /> Report to be submitted by December 7, 2001. A late start in completing this lateral and vertical <br /> investigation could jeopardize your ability to meet this deadline also.
