SAN .10 A Q U I N Environmental Health Department <br /> OUNTY <br /> Complaint Record Daily Time and Activity Summary from 5/24/2017 to <br /> 7/12/2017 for C00043527 located at <br /> 6649 EMBARCADERO DRIVE <br /> Activity Date Recorded by <br /> 05/30/2017 Completed the Emergency Response Record and corrected the photo log PE 2546 FLORIDO <br /> 24 min SC 544 <br /> 05/31/2017 Received a voice message from Tim Fontaine, of Village West Marina regarding PE 2546 FLORIDO <br /> 36 min the spill incident. SC 544 <br /> I called Tim back, he was not in the office. Left him a message. Tim called me <br /> back stating that the boat, the contaminated absorbent pads, and the bucket were <br /> no longer at the marina. He stated he would contain the boom that was left behind <br /> for the owner to dispose of. <br /> I called Fred of J& H Marin who said his crew took the boat out of the water on <br /> 5/28/2017. 1 told him that the contaminated absorbent and the bucket were no <br /> longer at the Marina. Fred said he gave his crew specific instructions to leave the <br /> used material at the Marina. I told him that the EHD needed the manifest for the <br /> used absorent. Fred said he would notify Erik Coster, who took out the boat and <br /> the absorbent from the marina. <br /> I spoke to the vessel owner, Greg Ebenhacker, who stated he would ensure Erick <br /> Coster obtained the manifest for the contaminated absorbent pads for EHD <br /> records. Greg also stated that the boom that was left behind was not contaminated <br /> and could be re-used, according to Erick Coster. I called Tim to verify Greg's <br /> statement regarding the boom. Tim was not in his office. <br /> 06/01/2017 Spoke to Tim, who said he is leaving the boom in the water for the owner to PE 2546 FLORIDO <br /> 36 min dispose of. SC 544 <br /> 1 left the owner a message that he must take out the boom, dispose of the boom <br /> at the Household Hazardous Waste Facility, and provide the EHD with disposal <br /> records. I also reminded him to provide the EHD with the manifest for the <br /> contaminated absorbent pads that were removed from the marina by J&H Marin. <br /> The owner called me back, but had not received my voice message. I repeated <br /> the above information to owner. I asked him to notify me when he plans on going <br /> out to the marina to retrieve the boom from the water. He confirmed he would <br /> notify me and that the boom would be out of the water and taken to the Household <br /> Hazardous Waste Facility within the next few days. <br /> 06/30/2017 Visited site to verify clean-up. Absorbent pads and boom were removed, clean-up PE 2546 FLORIDO <br /> 72 min verified. Received disposal record for absorbent booms from Greg Ebenhack. Still SC 544 <br /> awaiting disposal record for absorbent pads. Emailed Greg to remind him to obtain <br /> the manifest from J&H Marin for the disposal of the absorbent pads. <br /> Created and printed photo log for the clean-up verification. <br /> 07/06/2017 Prop 65 PE 2546 FLORIDO <br /> 30 min SC 203 <br /> Page 2 of 3 <br />
