SANJ O A Q U I N Environmental Health Department <br /> Complaint Record Daily Time and Activity Summary from 3/1/2015 to <br /> 6/21/2034 for C00043432 located at <br /> 6649 EMBARCADERO DR <br /> Activity Date Recorded by <br /> 05/08/2017 At 08:35am I was given the spill report for this incident. The spill report( CTRL# PE 2546 SAEED <br /> 336 min 17-3292 stated that"Historical Report*** Per NRC#1177728, "Caller is reporting SC 004 <br /> an unknown sheen in the Village West Marina. This sheen has been present since <br /> the incident date. Caller stated that there is a waste water treatment plant about <br /> 200 yards away and that it could be the source of the sheen". The spill report was <br /> initially completed by NRC and forwarded to Cal OES. The spill report indicated <br /> that incident date is 05/01/2017. <br /> At 09:00 am I called NRC and spoke to Keith Wather, and he confirmed the <br /> information on the spill report. I asked Keith that If I have questions about the <br /> specific location of the spill, would he be able to call the reported party and get the <br /> information. Keith told me the caller is Anonymous and he will not be able to do <br /> that. I explained to Keith that I am not asking for the caller phone number , and I <br /> just need their help to locate the sheet; Keith told me he can give me only the <br /> Coast Guard information and I can follow with them. <br /> At 09:08 am I called Cal OES and I spoke to Kristi and she told me that the report <br /> came from NRC , and she does not have additional information. I asked her if <br /> there was a spill report on 05/01/17 in regards to this spill, and she looked and she <br /> could not find any other reports beside this current report. <br /> At 09:17am I looked in CERS for West Village Marina's Environmental contact. I <br /> called the Environmental contact, Tim Fontaine. Tim told me he does not know <br /> anything about this spill, and nothing was reported or observed in regards to this <br /> spill. He told me there was no sunken boat or spill that he is aware of. He told me <br /> that Coast Guard called him earlier and he drove around and he did not observed <br /> the sheen. He mentioned at the Marina has security guard on site 24/7 and no <br /> spill was reported. He mentioned that last night there was a crush between two <br /> boats at 14 mile slough that resulted in minor scratches only, but there was no <br /> leak or spill reported. <br /> At 09:27am I discussed the conversation I had above with Elena, and she told me <br /> I will need to respond to verify. <br /> At 09:31 am I called Coast Guard and I spoke with Officer Alex Lee at <br /> (510)697-9664. Officer Lee did not have any additional information; I asked him to <br /> call me if he receives any updates. I got the contact information for Fish and <br /> Wildlife from officer lee. I was given Angel Tapia phone number(707)430-1357. 1 <br /> called Angel and he also did not have additional information; I told him to notify if <br /> they are responding or getting any updates. <br /> At 11:40am I filled out short term compliant form and submitted. At 11:49am Tim <br /> Fontaine called me and told me that he walked around the marina and was able to <br /> observed the sheen on water surface(300 X 50 Ft), and placed absorbent booms <br /> to contain the sheen. Tim told me that he does not know the source of the sheen, <br /> but he thinks it is motor oil, which may spilled from a container(unknown source). <br /> I told Tim I will be on site around 1:00pm today to inspect. <br /> At 01:00pm I arrived on site and met with Tim. Tim and I inspected the docks. <br /> Sheen was observed on docks A,C, D, E and F. The first 4 slips for all these <br /> docks showed a film of sheen. There was no Petroleum odor. At 1:30pm Haley <br /> and Rayan Rodriguez with department of Fish and Wildlife arrived on site. We <br /> continued the inspection, and we were not able to locate the source. We thought <br /> the sheen is vegetable oil, and it may came from the restaurant, but the Haley did <br /> the separation test for Oily water and they conclude it is not vegetable oil. We <br /> inspected the gust dock next to the restaurant and did not have any sheen. We <br /> interviewed the citizens who owned the boat, but did not know the source of it, but <br /> it was indicated that the sheen was traveling 5 days ago. We continued the drive <br /> and walk around the marina , but could not locate the source. At 03:10pm we all <br /> agreed to leave the site, and if there is any update, Tim will notify us. At 03:12pm I <br /> called Muni and updated him about the incident. I asked Muni if we billing for this <br /> Page 2 of 4 <br />
