(AGE) is hereby granted permission to perform a subsurface <br /> investigation at 101 West Beverly Place, Tracy, California (the site). The subsurface investigation <br /> is being conducted to assess soil and ground water for contaminants related to former operations at <br /> the former Church Refrigeration facility at 104 West Beverly Place, Tracy, California. The <br /> investigation is being conducted under the oversight of the San Joaquin County Health <br /> Environmental Health Department (EHD). <br /> Investigatory activities to be conducted on the site will consist of advancing two soil borings to a <br /> depth of approximately 204feet bsg, and two soil borings to a depth of approximately 9 feet bsg to <br /> collect soil and ground water samples, and may also include later installation and periodic <br /> monitoring of ground water monitoring wells in or near the soil boring locations. At the conclusion <br /> of the investigatory activities, all borings and ground water monitoring wells (if required) will be <br /> properly abandoned under permit with the original grade restored and the surface properly repaired <br /> to preexisting condition. <br /> All expenses for the subsurface investigation as related to the former activities at the 104 West <br /> Beverly Place site as outlined above shall be borne by Mr. Weldon Church, and will be conducted <br /> under permit in a timely manner upon approval by the EHD. <br /> Mr. and Mrs. MacDonald and all affected tenants of the site will be given 48 hours notice prior to <br /> the initiation of field activities, which will be conducted in a manner to minimize disruption of <br /> normal site activities. <br /> ZZ <br /> Mr. A Patterson, Jr Date <br /> Property Owner <br /> 101. virest Beverlv Place <br /> Tracy; California <br /> and/or <br /> A �-� Iz� o <br /> Mr. Melda Patterson Date <br /> Property Owner <br /> 101 West Beverly Place <br /> Tracy, California <br /> Advanced GeQEnvirorrn"tal,Inc. <br />
