The work pian is <br /> approved with the following conditions. Vertical delineation is required so drilling deeper <br /> than 50 feet below ground surface may be necessary. The bore holes will be grouted <br /> with Portland cement or bentonite grout. <br /> Submit a PHSIEHD permit application and $89 permit fee-for the borings. The work <br /> must be performed by a well driller with a C57 license and the permit will be issued to <br /> the C57 licensed driller.
