i <br /> Hazardous Materials Incident Report <br /> September 6, 1995 <br /> Page 3 <br /> SEQUENCE OF EVENTS: <br /> A Boyett Petroleum, 3 axle tank truck, pulling a two axle tank trailer, loaded with 8,810 i <br /> gallons of gasoline was eastbound on Mariposa Road, east of Murphy Road. The tank <br /> truck driver drifted off onto the dirt/gravel shoulder of the road, overcorrected to the left, <br /> crossed the center line and collided with a westbound Toyota pick-up. The impact <br /> caused the pick-up to be knocked onto the north shoulder and the tank truck's cargo <br /> tank was knocked off its frame and the tank came to rest across Mariposa Road. The <br /> tank trailer rolled onto it's right side and came to rest on the west shoulder of the road. <br /> The Toyota driver was killed on impact and was declared deceased at the scene by the <br /> San Joaquin County Coroner's deputy. The tank truck driver received moderate injuries <br /> and,was.,transoorted to the San Joaquin County Hospital for treatment. A salvage <br /> pump truck was called to the scene and 500 gallons of gasoline was recovered, the <br /> remaining 8,290 gallons spilled onto the roadway and into the drainage ditch on the <br /> west side of the road. Due to the extensive fire danger the roadway was closed from <br /> 0600 hours on 9-6-95 until clean-up was completed on 9-8-95 at 2100 hours. San <br /> Joaquin County Public Works Department established detour routes. The area was <br /> declared safe by the San Joaquin County Public Health Department at 1300 hours, on <br /> 9-6-95. Due to the remote location of the collision scene, the safety of the public was <br /> ensured by closing the roadway. An encroachment permit was issued to the Ramos <br /> Environmental Engineering firm and they performed clean-up services. On 9-8-95 at <br /> 2100 hours the San Joaquin County Health Department determined the hazardous <br /> materials had been removed and re-opened the roadway to the public. <br /> ACTIONS TAKEN: <br /> 1. Incident Command System (ICS): k <br /> A command post was set up on the west end of the accident scene on Mariposa <br /> Road. A unified command structure was put in place. Safety Officer: Captain Dave Luna, Farmington Fire Department. <br /> B. Medical Officer: Paramedic, Charles Schantz, Escalon Ambulance Service. <br /> C. Operations Officer: Chief Don Briggs, Farmington Fire Department. <br /> 1. Staging Manager: Officer Rick Okasaki <br /> 2. Traffic Coordinator: Peter Lepisto, S.J. Department of Public Works. <br /> 3. Fire Suppression: Chief Ed Martel, Montezuma Fire Department. i <br /> 4.
