Foley, REHS, Program Coordinator <br /> WN=Willy Ng, Senior Registered Environmental Health Specialist <br /> I <br /> 2120107 RVF <br /> 6:23 pm Received a request for assistance from the City of Stockton Fire Department <br /> (COSFD). This facility had tenants getting sick from an unknown Indoor air quality problem. One <br /> person had gone to the hospital for difficulty with breathing. The affected area had been isolated <br /> to the first floor, south wing. This was the second time this week that the SFD had been onsite. ' <br /> The COSFD had used handheld monitoring devices and got a hit for nerve agents(wintergreen oil) <br /> in the 45-55 range. There was not any spill and all of the cleaning agents used by staff had been <br /> the same. <br /> Onsite 7:15 pm. Met with Steve King, COSFD. Went inside the building. I could not detect any <br /> strange odors. Mr. King took me to the janitor's closet. There was a strong odor when the opened <br /> the door. Mr. King showed me aerosol cans that are used for an automatic air freshener device. <br /> When they first tested the building they got hits for nerve agent from his device. They requested <br /> that they discontinue use of this device to the staff. I asked if any water leaks had occurred <br /> recently. They showed me a spot in the lobby in front of the reception desk that had staining on <br /> the carpet. I met w/Rozann (night manager) and Melissa Soria (Day Manager)who stated that a <br /> large leak from the second floor had occurred in June 2006 and flooded the lobbyw/several inches <br /> of water. I detected a sheet rock patch in the ceiling. Rozann stated that they are going to have <br /> the carpets cleaned tomorrow. I gave her a handout of a list of Indoor Air Quality Consultants from <br /> the Department of Health Services. She stated that most of the management was at a conference <br /> in Long Beach. COSFD stated that they also scanned the mailboxes on the floor and they were <br /> clean. <br /> left the scene at 7:40 pm. <br /> 2121107 RVF <br /> Left a message w/Ruga Basu (510) 622-3156 w/ the Office of Environmental Health Hazard <br /> Assessment (OEHHA) regarding the investigation for a call back on any help they could provide. <br /> 2121107 KLF <br /> Approximately 10:30 am Received call from Captain Rodriguez of the City of Stockton Fire <br /> Department regarding a chemical release at above location. Ray von Flue of San Joaquin County <br /> EHD (EHD) responded to a similar complaint last night and he recommended the facility hire a <br /> Certified Industrial Hygienist. I asked Captain Rodriguez what he would like from EHD and he said <br /> he was just notifying us that the COSFD is receiving additional complaints but he didn't need EHD <br /> to respond. I thanked him for the call. <br /> 11:30 am Received another call from COSFD requesting EHD respond to the scene. I spoke <br /> with Deputy Chief Hafey and asked what he would like EHD to do at the scene. Hafey explained <br /> that the COSFD was evacuating a portion of the building and posting that section to prevent entry ' <br /> because he stated, "a resident from that building went to the hospital with an irregular heart beat <br /> x <br />
