BROADWAY AVE <br /> STOCKTOK CA <br /> APN: 143-150-08&APN: 143-150-22 <br /> Mr. Snider, <br /> On July 23, 1999 San Joaquin County Public Health Services Environmental Health Division (PHS/EHD) <br /> received and reviewed a site investigation workplan for the facility referenced above from Advanced <br /> GeoEnvironmental, Inc.on behalf of Western Square Industries Inc.dated May 20, 1999. <br /> No permits from PHS/EHD are required for the bucket auger excavation activities. A borehole permit is <br /> required for hollow stem auger,push-point sampling or similar subsurface investigation activities. <br /> PHS/EHD is requesting that amendments be made to the May 20, 1999 workplan following USEPA — <br /> Region 9 "Guidelines for Closure of Shallow DMosal Wells" (1992). <br /> The workplan lacks the following: <br /> I A plan view of the site showing the approximate location, connecting points, and length of the <br /> drain lines to and from the disposal wells. The plan view must also show the proposed sampling <br /> and borehole locations. <br /> 2 PHS/EHD recommends that samples be collected near the location of the former clarifier. <br /> Please submit an amended workplan to PHS/EHD no later than August 15, 1999. Please submit all <br /> correspondence to Ronald Rowe, Senior REHS.
