BROADWAY AVE <br /> STOCKTON,CA <br /> APN: 143-150-08&APN: 143-150-22 <br /> Mr. Snider, <br /> On May 13, 1999 San Joaquin County Public Health Services Environmental Health Division (PHS/EHD) <br /> received correspondence from Western Square Industries Inc. dated May 17, 1999 responding to a <br /> PHS/EHD request for information as it relates to the documented wastewater disposal wells at the above <br /> referenced facility. The May t7, 1999 correspondence was not signed although the information was hand <br /> delivered to PHS/EHD offices by you on behalf of Western Square Industries, Inc. An authorized <br /> representative of the company should sign all future correspondence submitted to PHS/EHD. <br /> Information supplied by Western Square Industries Inc. indicates that the disposal wells at this facility were <br /> installed approximately three years ago. No copies of disposal well construction or waste discharge <br /> permits for this facility were provided by May 19, 1999 as requested by PHS/EHD. Wastewater discharge <br /> permits are issued by the Central Valley Regional Water Quality Control Board(RWQCB-Region 5). <br /> PHS/EHD requests your cooperation in disclosing the names of the contractor(s) or individual(s) <br /> responsible for the installation of the disposal wells. PHS/EHD also requests that you describe how <br /> wastewater from your facility was disposed of prior to the use of the disposal wells(Le. - septic system or <br /> direct to land surface or subsurface). Please respond in written format before June 18, 1999. <br /> Following a review of the information submitted, PHS/EHD has determined that the disposal wells at the <br /> above referenced site were installed and operated without approval from any authorized permitting agency. <br /> San Joaquin County Development Title and Sewage standards clearly restrict the discharge of treated or <br /> untreated industrial wastewater to land surface or the subsurface. Laboratory analyses of wastewater and <br /> sludge dated April 30, 1999 indicate the presence of metals, oils, and grease in fluid waste streams <br /> potentially injected into the subsurface via the on-site disposal wells. <br /> PHS/EHD now directs you to properly close the disposal wells at your facility in accordance with <br /> federal, state, and local regulations. Please submit a closure workplan to PHS/EHD for review and <br /> approval before 5:00pm, July 25, 1999. The workplan should address the removal of the contents from <br /> the drains, drain lines, and the wells and determine appropriate disposal methods based on the results of <br /> sample analyses. The owner/operator is ultimately responsible for proper disposal of all wastes, and should <br /> carefully review all arrangements for disposal to ensure compliance with federal, state and local regulatory <br /> requirements. <br /> A Division of San Joaquin County Health Care Services <br />
