Field Investigation Work Plan <br /> Brookside Estates,Stockton, California 1/9/07 Sert Consulting, Inc. <br /> 4.2.4 Sample and Location Naming Convention <br /> P � <br /> ' The following sample identification system will be used for labeling all samples for laboratory use and <br /> data management. <br /> Sample type identifier: SS= Soil Sample,GW=Groundwater Sample <br /> SERT Subdivision Location Number: 1 =Brook Hollow, 2=Nostalgia,3 =Poppy Hills,4= Spyglass <br /> Sample Type Descriptor(SB)=Soil Boring <br /> ' Groundwater(GW) <br /> Unique Sample Location Number(SN) <br /> ' 4.2.S Sample Labelin,-Procedures <br /> Groundwater samples will be placed in containers provided by the analytical laboratory. A label will be <br /> firmly attached to the container side(not lid). The following information will be legibly written with <br /> indelible ink on the sample label attached to the sample container side: <br /> • Sample identification number <br /> • Preservation method, if any <br /> • Analyses to be performed <br /> s Sampling date <br /> ' • Sample time <br /> • Sample collector's name or initials. <br /> 4.2.6 Chain-of-Custody Procedures <br /> A chain-of-custody record is required for each shipment of samples. The form will be completed with all <br /> applicable information for each sample in the shipment. The record will be completed in indelible ink. <br /> Changes or corrections to the record will consist of line-out deletions(e.g., no"white-out" correction <br /> fluid)which are initiated and dated by the author of the change or correction. A copy of the chain-of- <br /> custody and other field documentation forms are provided in Appendix A. <br /> The chain-of-custody record will be completed by the field personnel who performed and/or witnessed <br /> the sample collection activity. After completion of the record down through the initial "Relinquished by" <br /> trow, a copy will be made of the record for retention by the originating field technician or designee. The <br /> original record will then be sealed in a plastic bag and secured in the shipping container. <br /> Samples will always be accompanied by a chain-of-custody record. When transferring samples,the <br /> individuals relinquishing and receiving the samples will sign and date the chain-of-custody record. <br /> Custody seals are not deemed necessary when the samples will be in continuous possession of technical <br /> or laboratory personnel. Custody seals will be used when samples are shipped via courier services. The <br /> chain-of-custody record will accompany each shipment. The method of shipment, couriers name(s), and <br /> other pertinent information will be entered in the chain-of-custody. <br /> 4.3 Field Quality Control Samples <br /> One duplicate groundwater sample will be retrieved from one location to measure the <br /> repeatability of the water analyses. <br /> 5.0 SCHEDULE \y�/ <br /> The anticipated schedule of the field activities described in this SAP during the period of January 14-1-x, <br /> 200V.
