Time In: 12:50 pm <br /> Time Out: 2:08 pm <br /> Food Program Complaint Inspection Report <br /> Name of Facility: JALOS TAQUERIA Date: 01/03/2018 <br /> Address: 25456 S SCHULTE RD,TRACY 95377 <br /> Owner/Operator: REYNOSO, HUGO ALBERTO Telephone: (209) 830-8066 <br /> Program Element: 1600- FOOD PROGRAM Complaint#: C00045338 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> C00045338 states: "COMPLAINANT ALLEGES ON 12/29/2017 ATAPPROXIMATELY 12:30 PM THEY ORDERED A <br /> CARNITA SUPER BURRITO TO GO. APPROXIMATELY 1 1/2 HOURS AFTER EATING, COMPLAINANT BECAME ILL <br /> WITH SYMPTOMS OF DIARRHEA. COMPLAINANT ALSO STATED THAT HALF WAY THROUGH THE BURRITO THEY <br /> FOUND A PIECE OF WIRE APPROXIMATELY 1 1/2"TO 2" IN LENGTH. PLEASE CALL COMPLAINANT WITH <br /> FINDINGS." <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #3 Employees with Discharge from Eyes, Nose, Mouth Major <br /> OBSERVATIONS:Observed an employee come from the back of the facility with blood on his lips and teeth. This employee <br /> shall be excluded from all food handling, exposed clean equipment, utensils, etc. until symptoms stop. <br /> Did not see employee for the remainder of the inspection. <br /> CALCODE DESCRIPTION:Employees experiencing sneezing, coughing, or runny nose shall not work with exposed food, clean <br /> equipment, utensils or linens.(113974) <br /> #6 Handwashing Facilities Supplied and Accessible <br /> OBSERVATIONS:The front of the house hand sink lacks paper towels in the dispenser and lacks liquid soap. Provide <br /> immediately. <br /> The back of the house hand sink lacks paper towels in the dispenser.
