Wilson, Stockton Program: HW SUMMARY OF VIOLATIONS (CLASS I, CLASS II, or MINOR -Notice to Comply) 113. No emergency coordinator assigned. At all times, there shall beat least one employee either on the facility premises or on call (i.e., available to respond to an emergency by reaching the facility within a short period of time) with the responsibility for coordinating all emergency response measures. This emergency coordinator shall be thoroughly familiar with all aspects of the facility's contingency plan, all operations and activities at the facility, the location and characteristics of waste handled, the location of all records within the facility, and the facility layout. In addition, this person shall have the authority to commit the resources needed to carry out the contingency plan. Submit evidence of compliance correction to EHD by 03/22/2014. 126. Information on the consolidated manifest receipt dated 9/20/13 (#106965) was inaccurate: Generator address and EPA ID# do not match. All copies of each receipt shall contain all the following information: (A) Name and address of the generator, and signature of the generator. (B) Date of shipment. (C) Manifest number. (D) Volume or quantity of each waste stream received, its California and RCRA waste codes. (E) Name, address, and identification number of the authorized facility to which the hazardous waste will be transported. (F) Transporter's name, address, and identification number. (G) The driver's signature. Immediately begin complying per this section. 128 (Repeat). No bills of lading for drained used oil filters before September 2013 were available for inspection. A copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all disposal records for the drained used oil filters for the last three years, maintain them on site, and submit copies to the EHD by 03/22/2014. 301. Witnessed oil and used absorbent spilled and leaked inside the gear room and truck repair shops (see Violation #108 above), and at the storage shelves under hanging engine parts. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately , and manage according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed by 03/22/2014. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
