Beckman Rd. Lodi, CA 95240 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> (2) Marking or labeling the individual item of UW with the date it became a waste or was received; <br /> (3) Maintaining an inventory system onsite that identifies the date the UW being accumulated became a <br /> waste or was received; <br /> (4) Maintaining an inventory system onsite that identifies the earliest date that any UW in a group of <br /> items of UW or a group of containers of UW became a waste or was received; <br /> (5) Placing the UW in a specific accumulation area and marking or labeling the area to identify the <br /> earliest date that any UW in the area became a waste or was received; or <br /> (6) any other method which clearly demonstrates the length of time that the UW has been accumulated <br /> from the date it became a waste or was received. <br /> Correct immediately by properly disposing the UW lamps with the accumulation start date of 3-14-13 <br /> and submit a copy of the disposal record the the EHD. For the other UW lamps, demonstrate when <br /> those UW lamps became a UW lamp or when the UW lamp was received. Continue to ensure that the <br /> facility demonstrates when a UW lamp became a UW lamp or when the UW lamp was received. <br /> Continue to ensure proper disposal of the UW lamp within a year of the lamp becoming a UW or <br /> receiving a UW. <br /> 999. HSC 25404(e)(4) Electronic submittal requirement on the California Electronic Reporting System <br /> (CERS) has not been met. Facility shall submit electronic information for hazardous materials <br /> management system, including Hazardous Waste, Aboveground Storage Tank and Underground <br /> Storage Tank on the CERS website which can be found at http://cers.calepa.gov/. Immediately comply <br /> with the CERS electronic submittal requirement per this section. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: <br /> ReredIBA <br /> B Title: <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
