The hazardous waste drum of metal fines lacked an accumulation start date. All hazardous waste <br /> containers shall be marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required <br /> information. Since it cannot be determined how long the drum of hazardous waste metal fines has been <br /> on site, immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest <br /> and submit a copy of the manifest to the EHD. <br /> 804. Five universal waste(UW) lamps were left leaning against the wall in the corner of the storage <br /> room. Three boxes of UW lamps were left open. UW lamps must be stored in a closed container and in <br /> a manner that prevents breakage and release to the environment. Correct immediately storing all UW <br /> lamps in a closed container and in a manner that prevents breakage and release to the environment. <br /> 808. Five universal waste(UW) lamps were left leaning against the wall in the corner of the storage <br /> room. Three boxes of UW lamps were left open. These UW lamps were not labeled as "Universal Waste <br /> Lamp(s)". Lamps (i.e., each lamp), or a container or package in which the lamps are contained, shall be <br /> labeled or marked clearly with the following phrase: "Universal Waste - Lamp(s)". Correct immediately <br /> by labeling the UW lamps on site and continue to ensure that all UW lamps are labeled properly. <br /> 809. Five universal waste(UW) lamps were left leaning against the wall in the corner of the storage <br /> room. Three boxes of UW lamps were left open. The tallest box of UW lamps had a start date of <br /> 3-14-13. These UW lamps have been on site longer than one year. The facility did not demonstrate <br /> when the other two boxes of UW lamps and the five UW lamps that were left leaning against the wall in <br /> the corner of the storage room became a UW or was received. <br /> (a) A UW handler shall accumulate UW for no longer than one year from the date the UW was <br /> generated, or was received from another UW handler. <br /> (b) A UW handler shall be able to demonstrate the length of time that the UW has been accumulated <br /> from the date it became a waste or was received. The UW handler may make this demonstration by: <br /> (1) Placing the UW in a container and marking or labeling the container with the earliest date that any <br /> UW in the container became a waste or was received; <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Re Title: <br /> Aris Veloso (209) 468-3453 <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
