Beckman Rd. Lodi, CA 95240 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> 804. Two universal waste(UW) lamps were left in an open container. A UW handler shall contain any <br /> lamp in a container or package that is structurally sound, adequate to prevent breakage, and compatible <br /> with the contents of the lamp. Such a container or package shall remain closed and shall lack evidence <br /> of leakage, spillage or damage that could cause leakage under reasonably foreseeable conditions. <br /> Correct immediately by managing all UW lamps per Title 22 Hazardous Waste Regulations. <br /> 809. The box of UW lamps had an accumulation start date of 7-8-11. These lamps have been on site for <br /> more than one year. A UW handler shall accumulate UW for no longer than one year from the date the <br /> UW was generated, or was received from another UW handler. Correct immediately by disposing of all <br /> UW lamps per Title 22 Hazardous Waste Regulations and submit a copy of the disposal receipt to the <br /> EHD by 8-10-12. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by 8-10-12. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Aris Cacapit (209) 468-3453 <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
