Lodi, CA 95240 Program: HW SUMMARY OF VIOLATIONS CLASS I, CLASS II, or MINOR -Notice to Comply) any lamp in a container or package that is structurally sound, adequate to prevent breakage, and compatible with the contents of the lamp. Such a container or package shall remain closed and shall lack evidence of leakage or damage that could cause leakage under reasonably foreseeable conditions. Correct immediately by managing UW lamps in a closed compatible container-. Minor -Notice to Comply. 83. The box of UW lamps was only labeled as waste. The container of UW batteries was only labeled as batteries recycling container. A UW handier shall label or mark UW to identify the type of waste. Lamps and batteries (i.e., each lamp and battery), or a container in which the lamps or batteries are contained, shall be labeled or marked clearly with the following phrase: "Universal Waste-Lamp(s) or Universal Waste- Battery(ies)". Correct immediately by labeling the UW lamps as "Universal Waste Lamps". Employee marked the batteries as Universal Waste Batteries. Class II Repeat Violation. 84. The box of UW lamps had an accumulation start date of 6-1-10. A UW handier shall accumulate UW for no longer than one year from the date the UW was generated, or was received from another UW handier. A UW handier shall be able to demonstrate the length of time that the UW has been accumulated from the date it became a waste or was received. The UW handler may make this demonstration by: (1) Placing the UW in a container and marking or labeling the container with the earliest date that any UW in the container became a waste or was received. (2) Marking or labeling the individual item of UW with the date it became a waste or was received. (3) Maintaining an inventory system on site that identifies the date the UW being accumulated became a waste or was received. (4) Maintaining an inventory system on site that identifies the earliest date that any UW in a group of items of UW or group of containers of UW became a waste or was received,- eceived;(5) (5)Placing the UW in a specific accumulation area and marking or labeling the area to identify the earliest date that any UW in the area became a waste or was received; or (6) Any other method which clearly demonstrates the length of time that the UW has been accumulated from the date it became a waste or was received. Employee acknowledged that the UW needs to be disposed within one year of the accumulation start date. The UW lamps have been on site longer than one year. Correct immediately by disposing of the UW lamps properly. Submit a copy of the disposal receipt to this office by 7-23-11. Class II Repeat Violation.
