Call 911. <br /> EMERGENCY MEDICAL CARE <br /> Minor: First-aid kits are available on site. <br /> Major: In the event of a major on-site medical emergency, medical services are available <br /> by calling 911. <br /> LOCATION OF NEAREST MEDICAL FACILITY: <br /> The nearest medical facility to the job site is Dameron Hospital at Harrison and Acacia Streets <br /> in Stockton, phone (249) 944-5550. Refer to the attached map for directions on how to get to <br /> the hospital. <br /> All injuries must be promptly reported to the Condor Health and Safety Officer (Ben White at <br /> Sonora office) and to the Principal-In-Charge of the lead office (Barry Hillman, Sonora.) <br /> In all on-site emergency situations, work will not resume until: <br /> 1. Conditions resulting in the emergency ha e been corrected. <br /> - 2. The hazards have been reassessed. <br /> 3. The Site Safety Plan has been reviewed. <br /> 4. Site personnel have been briefed on any changes to the Site Safety Pian. <br /> 5. The Site Safety Officer shall critique the emergency response and follow-up. <br /> LOCATION OF THE NEAREST TELEPHONE <br /> There are no permanent telephones available at the site. Warren E. Gomes Excavating, Inc. has <br /> a cellular telephone available in Mr. Gomes field vehicle.
